Heart Failure is a chronic and progressive disorder that impacts millions of our patients every year. This one disorder affects both young and old, and for some makes walking to the living room feel like a triathlon. Despite recent advances in medication, and quality of treatment the main challenges we face in 2016 are being able to make educated predictions on when our patients will return to the ER and understanding opportunities to intervene.

The Heart Failure Management Conference will address the latest cardiovascular risk reduction and disease management innovations for reduced readmissions and improved quality of life.
